Enter ANEURISM IV, a sprawling metropolis where the proletariat works without question and the air is thick with decay. As a Spirit—a spectral entity on the verge of becoming flesh and bone—you stand at a crossroads. The city is yours to influence, disrupt, or simply observe. With no set goals or rules, how you engage with this world is entirely up to you.

From the hardworking Prole to the affluent Banker, each fate offers unique abilities, challenges, and ways to interact with the city and its inhabitants. Your choice defines your journey:

  • Align with the Cortex to stabilize and protect the city.

  • Join the Scumbags or Zealots to spread the Rot and sow chaos.

  • Remain Neutral and immerse yourself in daily life, working in factories or exploring the city.

An ever-present threat, the Rot visibly alters the city as it spreads. Roles like Liquidators and Scumbags are pivotal in either containing or exploiting this menace. If left unchecked, the Rot will consume everything, leading to the city's demise.

ANEURISM IV is a sandbox simulator first and foremost. The city reacts dynamically to player actions:

this game is actually really good but in its current state its designed in a way that 14 year olds who just got done watching good griefs get to have power over you and silent try hards use you as anamnacyte farm without even saying a word. i dont want admins i dont want snitch culture but you know what else i dont want? my time wasted. having losers and freaks have complete advantage isn't fun, the little disclaimer in the beginning is really cute and edgy but your design choices embolden the worst, game killing type of people. you made an amazing game, now stop wasting people's time by design. Edit: So I checked out some other reviews and it made me realize im not crazy and there's another huge problem with this game: the developers. it sucks that the game is good, because the people that run it obviously don't know what the fuck they're doing or dont care. Extreme prevalence of very sexist people who will kill you for using voice chat if you are a woman. But really if you only type to respond to people you'll be killed while typing. Most I do is use this game to sell the cosmetics now since I can't even communicate without dieing half the time.

They just fired the community manage AyAyRon, so that's great. Except before he was fired he said this: "Based on what I saw in my role, Aneurism IV is complete but will receive updates and service for a very long time. There has and always will be 3 maintainers of Aneurism IV" And then they nuked all talk of Aneurism IV from their Vellocet discord. Pave the way for their next game, (Insert name of another "complete but unfinished" game). EL. OH. EL.

BIGGEST RUGPULL OF 2025 - DO NOT BUY This game is not what it used to be, it's not what you saw on Youtube, and it's not what I would recommend anyone buying. The inexperienced and misled devs are killing this game and you should not spend more than 50 cents on it until it gets better. I spent €10 for a light RP dystopian game with emergent gameplay. I got a sociopath simulator, which is actually much cooler than that. But for the last couple months to this date, the devs have been killing everything that made the game fun, and the community is vocal about non-issues that end up getting non-fixes because the devs listen to the crybabies instead of addressing the real problems. The game has turned into NPC simulator. The gameplay loop is boring and tedious. You cannot kill another player unless in very specific circumstances, and there used to be lots of thieves for that specific reasons, which had zero consequences for stealing. As the game grew, the devs cultivated the worst part of the player base, and now I don't ever want to think about playing this game again. The tagline of no moderation once meant no power-hungry admins and you were free to roleplay in a dystopian sandbox with jobs based around map exploration and human interaction. Now, whenever I play this game I am met with racial and ethnic slurs in a game where RP interactions have been exchanged for Hype Moments and Aura or complete targeted hatred towards minorities. The most alienating part of this game should be its setting and harsh game mechanics, now players who would enjoy the design of this game won't even play for longer than half an hour before quitting and refunding. The modding scene was sacrificed for stopping cheaters (which didn't even work), the Discord server has the same issues of rampant hate speech (while having a rule against gay fanart of their genderless playermodels you're supposed to make into your own characters), and jus...

This game is a disgusting, grimy, festering wound of a game, and I love it. If you enjoy rps or social sims, you might enjoy this game. It definitely isn't for everyone, but its the only game of it's kind and I cannot recommend it enough. Remember this though, the rot never stops, and it festers even in the Cortex. Be kind to your fellows. We're all dying anyway, at least make sure you're dying with friendly hands clasped and not voracious hands grasping.

I have never wasted more time accomplishing absolutely nothing in my life. Game tells you how to nothing, the wiki doesn't either. I've looked all over on google and reddit and not a soul in the game will tell you how to do anything paste the basics.
